INITIAL CONSULTATION

PLEASE ENSURE YOU HAVE PROVIDED:

  • A valid referral from your General Practitioner or relevant specialist. (Without this we will not be able to see you).

  • All relevant examinations (i.e. X-rays, MRI scans, EMG studies, etc.) Please ensure you have the film or discs of any relevant radiological examinations.

  • Your Medicare and Private Health Insurance Card or

  • Workers’ Compensation Details - the name and contact number of your employer, the name of the insurance company that is managing your claim, and your claim number. 

APPOINTMENTS

Whilst we aim to run on time, Mr Keogh is a trauma surgeon, and clinically urgent referrals will be accommodated and prioritised. You are most welcome to call on the day of your appointment to check if there are any unexpected delays.

PARKING

Paid parking is available in the underground multi-level car park at the Subiaco Clinic, entry via McCourt Street and the multi-level carpark at the main hospital entrance, entry via Salvado Road. 

Street paid parking is also available in the surrounding streets around the clinic / hospital.
